Should PrEP Be Available As An Over-The-Counter Medication?

Yesterday I reported on a push by California lawmakers to make PrEP (pre-exposure prophylaxis) and PEP (post-exposure prophylaxis) available to the public as over-the-counter medications to be dispensed by a pharmacist.

The legislation, Senate Bill 159, has passed in the state Senate and is working its way through the Assembly.

The bill has strong support from the San Francisco AIDS Foundation, APLA Health, the Los Angeles LGBT Center, and the California Pharmacists Association.

According to the Centers for Disease Control(CDC), studies have shown that PrEP reduces the risk of getting HIV from sex by about 99% when taken daily. So making the medications more accessible definitely sounds like a good idea.

But there are those who have reservations about the bill.

The California Medical Association (CMA) wants to see changes made before the bill becomes law. The CMA points to possible drug interactions as well as complications with liver or kidney functions that might occur if the patient isn’t being monitored on a regular basis by a physician.

There are other concerns as well.

For instance, Gilead Sciences, which manufactures the drug Truvada (tenofovir) used in PrEP, was sued in April by eight patients who took the medication for years.

They say Gilead “deliberately withheld a much safer version of these drugs from the market for more than a decade” in an attempt to maximize profits from Truvada, according to Biospace.com.

The plaintiffs say Gilead was slow in sharing adverse event reports which indicated HIV drugs using a formulation known as tenofovir disoproxil fumarate (TDF) caused far more serious complications than previously reported.

Medications that utilized this formulation were marketed up to 2015 as Truvada, Viread, Atripla, Complera and Stribild.

Gilead Sciences had developed a safer, more effective anti-viral HIV treatment formulation, tenofovir alafenamide fumarate (TAF), which was patented in 2015.

While the lawsuit is slowly working its way through the courts, some critics of SB 159 say the general public may not know which medications may or may not work for them. Plus, would the average pharmacist have an extensive background on the different medications used for PrEP?

Additionally, Gilead is on the verge of having a new HIV drug, Descovy, approved for use in PrEP. Descovy has already undergone phase II trials and is awaiting final approval which could come as soon as September or October.

While Truvada continues to be effective, according to Biospace.com, Gilead says Descovy is “safer than Truvada for PrEP and ‘offers more efficient delivery’ of the antiviral components, which results in a higher concentration of the drug in the cells where HIV can occur.”

Gilead has stated an approved version of Descovy would cost about the same as Truvada, about $21,000 a year.

While that FDA approval is expected shortly, an Israel-based company, Teva Pharmaceutical, is expected to make a generic version of Truvada available possibly as soon as 2020.

Since 2004, Truvada has definitely helped save lives as part of a daily HIV treatment and, since 2012, as the only drug approved for use as PrEP.

I personally support making safe, life-saving drugs as available as possible.

But here’s a question – if you knew there was a less-toxic but more expensive drug on the market versus an older, generic medication, what would you do?

Wall Street experts say increased stock market volatility may be signs of an impending recession as Donald Trump’s trade war with China continues to heat up.

Over just the last few days, economists at Goldman Sachs, Morgan Stanley and Bank of America all warned that Trump’s bitter trade war with China is taking a bigger bite out of economic growth than expected.

The warnings came as stocks suffered another big dip on Monday with the Dow closing off nearly 400 points, or 1.5 percent, putting the blue-chip index at 25,897, over 700 points lower than it was in January of 2018 before Trump’s trade fights began in earnest. Stocks bounced back Tuesday after the Trump administration’s announcement about postponing some tariffs that had been set to take effect next month, continuing a long track record of market volatility tied to trade policy.

The collective wisdom now spreading across Wall Street is that no trade deal will be struck with China before the 2020 election; business investment will continue to sag; and a series of interest-rate cuts from the Federal Reserve won’t be enough to juice more growth out of an economy now in its tenth year of expansion — the longest stretch in American history.

With economic growth slowing to 2.1 percent in the second quarter, Trump is predictably shifting from crowing about the stock market to blaming the Federal Reserve for not utilizing more interest rate cuts.

Bank of America’s head of U.S. economics Michelle Meyer said in a note to clients that she believes there is a 1-in-3 chance of a recession in the next 12 months.

CNN anchor Chris Cuomo got drawn into a heated verbal exchange when an unidentified man in New York approached Cuomo in public.

Cuomo was out with his family as the man addressed him and repeatedly called him, “Fredo.” The name references the Fredo Corleone character in the movie, The Godfather, who was the weak brother who betrayed his family.

According to Cuomo, who is proud of his Italian heritage, being called “Fredo” is an ethnic slur.

Apparently, right-wing fanatics call Cuomo by the name to denigrate the newsman for his reporting on Donald Trump.

The anonymous man insists he thought Cuomo’s name was “Fredo,” but I think that’s a bit disingenuous.

The far-right nuts are living for this.

Cuomo admittedly gets riled up, but considering he was with his family and the man reportedly continued to badger him, I think it’s understandable. There are no reports that the incident escalated to physical violence.

CNN has made a statement saying they stand with him, and Cuomo has tweeted an acknowledgement he could have handled the incident better.

Even conservative figures like Fox News pundit Sean Hannity and former White House communications director Anthony Scaramucci weighed in saying that the unidentified man was a jackass for trying to bait Cuomo while out with his family.

Virginia Restaurant Ordered To Pay $40K For Harassing Gay Server

A Virginia restaurant operator has agreed to pay $40,000 to resolve a lawsuit that claims its employees routinely harassed a gay server with homophobic epithets and taunted him about his sexuality.

A federal judge on Thursday signed off on the settlement between the U.S. Equal Employment Opportunity Commission and Mejia Corp., which does business as El Tio Tex-Mex Grill restaurant in Gainesville.

The commission’s September 2018 lawsuit claimed restaurant employees also harassed the gay server’s heterosexual friend, a busser, based on their friendship.

The settlement calls for the server and his friend to split the $40,000 payments from the company. The agreement also bars the restaurant from engaging in or condoning sex-based harassment of any employee.

The EEOC determined that the two men had legal standing to sue for discrimination under Title VII of the Civil Rights Act of 1964.

Recent court cases have found that discrimination based on sexual orientation is covered under the prohibition of “sex” discrimination under Title VII.

California: PrEP And PEP Could Become Available Without Prescription

California lawmakers are working to make PrEP and PEP available as over-the-counter drugs without a prescription, reports Capital Public Radio.

PrEP, pre-exposure prophylaxis, is the once-a-day treatment using Truvada that has been proven to be more than 90% effective in preventing HIV infection.

PEP, post-exposure prophylaxis, is prescribed to patients who have experienced possible exposure to HIV. Taken within 72 hours of exposure, the 28-day course of HIV medications is meant to stave off infection before the virus can take hold in the patient’s system.

Both PrEP and PEP currently require a prescription from a physician. Often, a patient’s insurance company has to sign off on the prescription before coverage kicks in.

California state Sen. Scott Weiner of San Francisco wants to change that. He is the lead author of Senate Bill 159.

The legislation would authorize pharmacists to provide PrEP and PEP without a prescription, much like birth control pills are currently available in the Golden State.

Under SB 159, Patients would have to be tested for HIV within the seven day period before buying the medication, and pharmacists would have to notify a patient’s primary physician as well as counsel the patient on side effects.

The measure passed in the state Senate in May by a vote of 34-1, and it is now working its way through the state Assembly where Assemblymember Todd Gloria is sponsoring the bill.

Equality California has made the bill a top priority. Additionally, the legislation is supported by the San Francisco AIDS Foundation, APLA Health, the Los Angeles LGBT Center, and the California Pharmacists Association.

But the California Medical Association wants to see changes made before the bill becomes law.

Specifically, the CMA wants the PrEP component removed from the bill, and only supports allowing a portion of PEP to be dispensed by a pharmacist before a doctor would prescribe the remaining treatment.

The CMA points to possible drug interactions as well as complications with kidney or liver functions that could occur if dispensed without regular checkups and screening.

In a letter to lawmakers, Morgan Carvajal, legislative advocate for the CMA’s Center for Government Relations, wrote, “Prescribing a daily, long-term use, medication of strong HIV regimen drugs to healthy, HIV-negative patients demands a high degree of safety and consideration that is only achieved safely by the patient-physician relationship.”

There’s no word as yet regarding pricing of the medications under SB 159.

Meet Chris “Big Bear” McNaghten, the openly gay Northern Ireland strongman who recently attended his first Pride event and is featured in the new BBC documentary, My First Pride.

McNaghten shares in the documentary that he is “the first openly gay strongman to come out in the UK and Ireland, possibly even Europe.”

The beefy 30-year-old shares other ‘firsts’ in his life as well, including coming out as gay.

McNaghten explains that coming out to one of his coaches, who had become a close friend as well, was one of his more difficult challenges to overcome.

“I was just as nervous about telling him about it as I was about my dad,” explains the Strongman champion. “It was a wee bit emotional. That was just a massive weight off my back.”

After the coming out process, it was finally time to attend his first Pride. So off to Belfast, Northern Ireland, he went with his fiancé, Jon.

The couple is engaged although same-sex marriage isn’t legal (yet) in Northern Ireland.

While the LGBTQ landscape in his home country hasn’t caught up with the dozens of countries around the world that have embraced marriage equality, he has seen how social attitudes have shifted in other countries.

Take, for example, public displays of affection.

“Whenever we have been in London together, there’s no issue with holding hands, there are no issues being in restaurants with each other or kissing or showing any type of affection with each other,” says McNaghten. “But back home, we have yet to do it.”

Still, even at home, beefy “Big Bear” does enjoy many of the same things LGBTQ folks do, like RuPaul’s Drag Race, which he sees as comparable to what he does in his sport.

“Drag is about putting on a performance and about performing for people,” he says. “It’s the exact same way whenever I’m on stage competing.”

And speaking of “the exact same,” McNaghten is looking forward to the day when his sexuality isn’t really a part of his fame in the sports world.

“I want to be Ireland’s strongest man,” he says. “I don’t want to be Ireland’s strongest gay man.”

The results of a new poll of head-to-head matchups between Donald Trump and possible Democratic opponents by SurveyUSA shows two front-runners would beat Trump by 8 points.

The poll of 5,459 registered voters nationwide showed:

• Former Vice President Biden 50%, President Trump 42%

• Vermont Senator Bernie Sanders 50%, Trump 42%

• Massachusetts Senator Elizabeth Warren 46%, Trump 44% — too close to call

•  California Senator Kamala Harris 45%, Trump 44% — too close to call

• Trump 44%, South Bend IN Mayor Pete Buttigieg 42% — too close to call

The polling company says that the survey “approximates the outcome of the USA popular vote but not necessarily the outcome of the Electoral College.”

SurveyUSA also notes that Trump doesn’t break 44% against any Democratic opponent.
